As a member of the CPHC, the ACFP continues to advocate for and support team-based care, a pillar of their parent organization’s vision—Patient’s Medical Home, by facilitating community- and relationship-building. The ACFP has donated in-kind space on their online community platform.
The Team-Based Primary Care community is a community of practice for interested individuals who want to learn, share, and advance team-based care in Alberta’s health system, including past participants of the Team Primary Care Alberta forum. This community will help continue the conversations and connections made at the TPCA and create new ones among engaged individuals.
