Team-based care can be implemented in various ways and there are many teams across Alberta currently delivering it.
The Coalition for Primary Health Care is looking to showcase various examples of team-based care within our province. With our Spotlight series, we hope to:
Simply fill out the Spotlight form below. Where possible, be as descriptive as best you can to tell your story. The CPHC Working Group will review submissions before publishing stories on the CPHC website.
Once published, the CPHC and its member organizations will do their best to share yours stories more broadly through their respective communications platforms. The CPHC may also invite teams to present their stories at a later date through a webinar series or at a Coalition committee meeting.
