As the first contact point to the healthcare system, primary care in Canada faces unprecedented pressures from provider shortages, long wait times, and the growing burden of chronic and complex conditions, leaving many patients without timely access to care. Team-based primary care, where diverse health professionals collaborate with patients and caregivers to deliver coordinated services, has emerged as a promising approach to improve access, quality, and efficiency while reducing provider burnout.
The objective of this rapid review is to identify and synthesize published and grey literature on national and international models of team-based primary care (TBPC), related outcomes, and key implementation considerations, including barriers, facilitators, and access.
