Team Primary Care Alberta Forum – Wrap Up

Successful implementation of team-based care in Alberta will rely on a common vision and strong working relationships among citizens, professional organizations, and partners. The Alberta College of Family Physicians (ACFP) proudly presented, in collaboration with the Coalition for Primary Health Care (CPHC), an invitational forum on advancing team-based care on October 4 and 5 in Calgary.

The ACFP worked collaboratively with the planning committee and the CPHC to ensure the program reflected the diverse perspectives of leaders in primary health care. Team Primary Care Alberta Forum (TPCA) hosted 170 interdisciplinary leaders and partners in primary care to learn and collaborate with each other. Many of the voices of primary health care were represented to consider how team-based primary care should look and feel. In the room and online, we had citizens, patients, caregivers, community representatives, family physicians, pharmacists, nurses, nurse practitioners, behavioural health consultants, social workers, clinic managers, primary care leaders, learners, and policy makers joining us to support a better way forward.
